Who Can Legally Inject Botox in Texas, and What It Costs
Texas rewrote its rules on cosmetic injecting in 2025, and the new version hands you something specific you are entitled to be told before anyone touches you. Here is what it requires, and what Botox costs across the three Texas cities we have verified.
Who may legally inject Botox in Texas
Injecting a prescription medicine such as Botox for cosmetic reasons is the practice of medicine in Texas. The Texas Medical Board keeps its rules on nonsurgical medical cosmetic procedures at 22 Texas Administrative Code §§169.25 to 169.29, which replaced the board’s older rule at §193.17 in 2025.
§169.25 treats “the injection of medication or substances for cosmetic purposes” as an act a physician may delegate and supervise, and that delegation has to satisfy section 157.001 of the Texas Occupations Code. What that means for you as a patient: a physician, or a physician assistant or advanced practice registered nurse acting under a physician’s delegation, has to establish the relationship with you, keep your medical record, and tell you who is actually going to perform the injection and what their credentials are, before it happens.
A 2025 bill that would have tightened who may inject, Senate Bill 378, was vetoed and did not become law, so the board’s delegation rules above are still what governs.

What the Texas price bands actually show
Three Texas cities are verified here and their bands overlap rather than sitting apart: San Antonio $10.50 to $12.50, Austin $11 to $13.50, Houston $12 to $14.50.
They do not track metro size. Houston is the most expensive of the three and it is also the largest metro, but Austin is dearer than San Antonio despite the Austin-Round Rock-San Marcos metro being the smaller of the two. Whatever sets the rate in Texas, population is not doing the work on its own.
The three Texas bands are also unusually tight, $2 to $2.50 wide each. Of the eighteen cities verified across this site, three of the five narrowest bands are in Texas. Texas clinics that publish a rate tend to agree with each other more closely than clinics in the other states covered here.
End to end the state runs $10.50 to $14.50 per unit. On the 20 unit glabella dose the FDA label specifies, that is $210 against $290 for the same treatment.
